A year into opening the doors to Cypress Optique in Cypress, Texas, I knew I wanted more. I had no interest in the traditional model of routine refractions and quick optical sales. I wanted an elevated, medical-grade approach to eye care.

Today, three years into my cold start journey, I lead a solo-doctor practice that has reached seven figures. This growth was entirely intentional, sparked by a strategic shift into specialized dry eye care. By investing in advanced technology and pouring into my team, I’ve transformed Cypress Optique into a referral hub that bridges the gap between medical eye care and the art of aesthetics.

FINDING THE TURNING POINT

Shortly after settling into Cypress Optique, my husband and I assessed our foundation. We had the footing and the patients, but we still had the capacity to scale into something more. I realized this was the moment to manifest the dream.

Dry eye was the natural choice. My background practicing in Boston and Denver—climates where ocular surface disease is rampant—taught me that dry eye isn’t a matter of if, but when. I decided to stop being reactive and, instead of waiting for a patient to report symptoms, evaluate for the disease before it escalates into a crisis. That was step one.

INVESTING IN THE TRIFECTA

To manifest the dream, I had to do more than start screening for dry eye. I had to invest. Providing palliative care was never my goal, so I did more than just stock a few drops on the shelf. I chose to invest in advanced treatment technology, starting with the Lumenis OptiLIGHT intense pulsed light (IPL) therapy.

Before the device even hit the clinic floor, the groundwork was laid. We initiated a series of drip email campaigns to our patients, sharing my passion for dry eye and the clinical reasoning behind our new technology. By the time the OptiLIGHT was delivered, we already had a roster of patients eager to book evaluations.

Six months later, we rolled out the Lumenis OptiPLUS dual-frequency radiofrequency device, and six months after that, the Lumenis Dynamic Muscle Stimulation technology. This suite of technology was strategically chosen to bridge the gap between medical necessity and aesthetic optimization. For my practice, this is the ultimate trifecta.

THE CASH-PAY CONVERSATION

Cash pay procedures are a big hurdle for many optometrists. We get stuck in the realm of comfort, feeling obligated to offer only what insurance covers. But insurance rarely covers the root cause of dry eye.

I believe in absolute transparency with patients from the get-go. I tell them this:

“Some of the treatments I’m going to offer you today are cash pay, meaning there is no insurance coverage. We can certainly use insurance-covered treatments, but those are often limited to basic symptom management. To address the underlying cause of your dry eye, I recommend our advanced clinical packages. It’s your choice how we proceed, and I’m here to facilitate whichever level of care you feel is right.”

Clinically and financially, the ROI was staggering. Both the OptiLIGHT and triLift reached full ROI in about four months, and the OptiPLUS in six. In a high-volume practice, that could happen even faster. Since incorporating this technology, my dry eye sector grew by 78% last year, with year-to-date growth hitting 102%.

EMPOWERMENT THROUGH DELEGATION

Expanding to a team of nine required me to release the reins of total control. I initially resisted delegation, anxious about how a patient might interpret a staff member performing a treatment. I wanted my hands on every part of the process, convinced that “my way” was the only way.

However, total control isn’t sustainable, especially for a seven-figure practice. I overcame this by pouring into staff education and empowerment. Today, I have a licensed aesthetician and clinical leads who perform nearly all treatments. Their hands-on training under my supervision means I can trust their clinical judgment and adherence to my brand’s standard of care, ensuring the practice runs efficiently without me as the bottleneck.

STRATEGIC REFERRALS AND OCULAR AESTHETICS

Even with this technology, scaling to seven figures required me to look beyond the four walls of my practice. I didn’t waste my time knocking on every door in town. I was strategic. I researched local specialists in oncology, gynecology and rheumatology—clinicians who see patients with systemic inflammatory and hormonal drivers of dry eye—then paid them an in-person visit.

I built a referral network based on trust and consistency. When a rheumatologist sees their lupus patient’s quality of life improve because of my dry eye protocols, it establishes a self-sustaining ecosystem of clinical trust and a recurring referral partnership. In one 60-day period, this strategic referral outreach generated $50,000 in revenue from specialized packages alone.

Perhaps the most surprising part of this journey has been how naturally aesthetics fit into my medical model. I initially approached this from a clinical perspective but quickly realized how much patients value the fusion of health and aesthetics. Patients are instantly engaged the moment I explain how we can address their clinical symptoms and aesthetic goals simultaneously. Addressing both functional health and aesthetics through a single technology suite is a win-win that builds profound patient trust and confidence in our specialty care.

A TWO-MINUTE CATALYST FOR CHANGE

To my colleagues considering this path: write down your plan. A goal in your head is merely a wish.

You must be willing to get uncomfortable. Whether you’re a brand-new cold start or a busy veteran, growth doesn’t happen by doing what you did last year. Step outside your comfort zone and start with small changes—a two-minute dry eye screener for every patient can change your trajectory.

And don’t do it for the money. The profit will follow the passion. If you stay true to your vision and provide the best care for your patients, the growth you imagine is inevitable.
